 Payment is due in full at the time of service.  Road Maps 2 Health currently does not accept any forms of insurance; however, payment in the form of cash, check, debit or credit card, Health Savings Account (HSA) or Flexible Spending Accounts (FSA) are gladly accepted. Superbills are available upon request; nonetheless, Road Maps 2 Health cannot guarantee that you will receive any reimbursement for our services, from your insurance company. 

 Please note: Suggested supplements, vitamins, nutrients, homeopathic remedies, herbs, tests and etc are optional and are a SEPARATE COST than the appointment. 

Supplements, vitamins, homeopathics, etc that are purchased in office are non-refundable.

 You and your appointment time are important! Road Maps 2 Health requires a 24-hour cancellation policy. Appointments cancelled with less than a 24-hour notice will be charged at the full cost. No shows will be charged the full amount of the missed appointment, as well. Cancellations and rescheduling must be made via email to the patient portal or office@roadmaps2health.com You will receive a confirmation email stating that your appointment has been cancelled.
